Bacillus coagulans is a lactic acid-producing bacterium that forms hardy spores. It was once sold as Lactobacillus sporogenes and now also appears as Weizmannia or Heyndrickxia coagulans. The research is strain-specific: the main studied strains are Unique IS2, MTCC 5856 (sold as LactoSpore) and GBI-30, 6086 (sold as BC30), and a result for one does not carry over to another. Randomized trials in people with IBS report less abdominal pain and bloating, and trials in adults with infrequent or hard stools report more regular bowel movements, but most trials are small and run or funded by the strain's maker, and a constipation meta-analysis found no clear effect on stool frequency for pooled Unique IS2 trials. Small studies also report higher blood amino acid levels when a strain is taken with protein. It is not a treatment for any digestive disease.

Benefits

Abdominal discomfort, bloating and stool form

In adults diagnosed with IBS, strain Unique IS2 (2 billion CFU daily for 8 weeks) eased abdominal pain and increased complete bowel movements versus placebo, and strain MTCC 5856 improved bloating, pain and stool frequency in a small pilot in diarrhea-type IBS. A strain-by-strain meta-analysis supported Unique IS2 but pooled only two trials, one of them in children; each strain's trials were run by its maker.

After-meal gas and abdominal discomfort in adults without a diagnosis

In 61 adults with after-meal gas but no digestive diagnosis, a product with strain GBI-30, 6086 improved abdominal pain and total gut-symptom scores more than placebo over 4 weeks. Bloating showed only a trend, and a strong placebo response meant many other measures did not differ between groups. This is a single small trial.

Stool frequency and consistency

Strain Unique IS2 increased bowel movements in 100 adults with functional constipation and reduced hard stools in 144 healthy adults with infrequent bowel movements, without changing overall gut symptoms in the second trial. Strain GBI-30, 6086 raised weekly stools in 111 healthy adults. A meta-analysis of pooled Unique IS2 trials, however, found no significant rise in stool frequency.

Studied as an add-on to oral rehydration in children

In 110 children aged 1 to 10 with acute diarrhea, strain MTCC 5856 given with oral rehydration and zinc for 5 days shortened diarrhea by about 11 hours versus placebo, though daily stool frequency did not differ. An earlier trial in 148 children aged 6 to 24 months using an unnamed strain found no effect. Diarrhea in young children needs medical care.

Amino acid absorption from protein

Taken with a daily protein shake, strain GBI-30, 6086 raised blood amino acid levels after the meal in two small crossover studies of 30 people each (milk protein, and rice and pea protein). Strain Unique IS2 taken with whey for 60 days raised blood amino acids in trained men, and the authors also report gains in leg-press strength. Apart from that strength result, these are blood markers from small, maker-linked studies.

Spores that pass the stomach and germinate in the gut

Bacillus coagulans forms tough spores. Maker-linked research on strain GBI-30, 6086 describes the spores surviving stomach acid and germinating in the intestine, where the bacteria become active. How many spores of a given product survive and germinate in people has rarely been measured directly, and it can differ by strain and formulation.

Lactic acid production

Once active, the bacterium ferments sugars into lactic acid. A lower local pH is a proposed way it shifts the gut environment. Trials have measured only small changes in gut bacteria, and one trial found no change in microbiota composition at all.

Help with breaking down dietary protein

Strain GBI-30, 6086 increased protein digestion in a laboratory model of the stomach and small intestine. The proposed idea is that germinated cells release protein-digesting enzymes that free more amino acids for absorption. Higher blood amino acid levels after a protein meal fit this idea, but the enzyme effect itself has not been measured in people.

136 adults aged 18 to 60 meeting Rome III criteria for IBS, randomized after a run-in that excluded non-compliant participants.

Both primary outcomes, abdominal pain intensity and complete spontaneous bowel movements, improved more than with placebo, as did secondary symptoms. Blood inflammatory markers did not change. No severe adverse events were reported. Three authors are employees of the strain's maker.

36 people newly diagnosed with diarrhea-predominant IBS (18 per group).

Bloating, vomiting, diarrhea, abdominal pain and stool frequency improved more than with placebo (p < 0.01), and quality of life rose. The trial was small and was run by the strain's maker, Sami Labs and Sabinsa.

144 healthy adults averaging 3 to fewer than 7 complete bowel movements per week.

Bowel movement frequency, the primary outcome, rose significantly (p = 0.037), and stool consistency improved more than with placebo (p = 0.018), mainly through fewer hard stools. There was no effect on GI symptoms, quality of life or gut microbiota. PepsiCo sponsored the trial, and three co-authors work there.

Randomized, double-blind, placebo-controlled trial of Bacillus coagulans strain GBI-30, 6086, 1 billion per day for 4 weeks (Gang et al. 2025, Benef Microbes).

111 healthy adults aged 18 to 65 with functional GI complaints that did not meet criteria for a disease.

Stools per week increased (p = 0.036), consistency improved (p < 0.001) and fewer participants had constipation versus placebo. Gut bacteria changed only slightly. Several authors work for Kerry, which sells the strain.

MTCC 5856 Added to Oral Rehydration in Children with Acute Diarrhea

Randomized, double-blind, placebo-controlled trial of strain MTCC 5856 (400 million spores) or placebo with oral rehydration solution and zinc for 5 days (Majeed et al. 2023, Front Pediatr).

110 non-hospitalized children aged 1 to 10 with acute diarrhea of any cause.

Diarrhea lasted 51.3 hours with the probiotic versus 62.7 hours with placebo (p = 0.011), and dehydration status improved more. Daily diarrhea frequency was lower but not significantly. Most authors are employees of the maker.

148 children aged 6 to 24 months with acute watery diarrhea and some dehydration (78 probiotic, 70 placebo).

Recovery rate, duration, frequency and volume of diarrhea, and rehydration fluid intake did not differ significantly between groups, including in rotavirus cases. The authors concluded the product had no therapeutic impact. This null result is one reason findings for one strain should not be assumed for another.

Unique IS-2 with Whey Protein in Resistance-Trained Men

Double-blind, placebo-controlled trial: 20 g whey protein daily with 2 billion CFU Unique IS-2 or with placebo for 60 days alongside a controlled resistance-training program (Tarik et al. 2022, Eur J Nutr).

Resistance-trained men averaging 21 years old.

Total plasma free amino acids rose 16.1% (p = 0.004), and branched-chain amino acids were 33.1% higher than with placebo at 60 days. The authors also report a probiotic effect on leg-press 1RM (+16.6%, p = 0.024) and vertical jump power (+7.9%, p = 0.007). One trial, with two co-authors from the strain's maker.

Side effects and drug interactions

Common Potential side effects

Well tolerated in the cited trials, which ran from 5 days to 90 days; in pooled IBS analyses, adverse events with Unique IS2 and MTCC 5856 did not differ from placebo. Longer-term use has not been studied in these trials.
Any new probiotic can change gas or bowel habits for a few days; stop and talk to a doctor if digestive symptoms persist or worsen.
People who are severely immunocompromised, critically ill, or have a central line should talk to their doctor before taking any live probiotic.
Diarrhea in infants and young children, or diarrhea with fever, blood or signs of dehydration, needs medical care. In children the probiotic was studied only as an add-on to oral rehydration.

Antibiotics may reduce the number of live bacteria. No cited trial tested timing, so taking the probiotic a few hours apart from an antibiotic dose is a common precaution rather than a tested rule.

Why does the strain on the label matter so much?

Probiotic effects are tested strain by strain. Unique IS2, MTCC 5856 (LactoSpore) and GBI-30, 6086 (BC30) are all Bacillus coagulans, but each has its own small set of trials, often on different outcomes. A pooled analysis found benefits for Unique IS2 in IBS, while an infant diarrhea trial of an unnamed strain found none. Look for a product that names its strain and matches the dose used in that strain's trials.

Is it the same as Lactobacillus sporogenes, Weizmannia coagulans or Heyndrickxia coagulans?

Yes, these are names for the same species. Older products and papers call it Lactobacillus sporogenes; it was later classified as Bacillus coagulans. Taxonomists have since moved it to the genus Weizmannia and then proposed folding that genus into Heyndrickxia, so recent papers use all three names.

Will it help with constipation?

The evidence is mixed. Strains Unique IS2 and GBI-30, 6086 increased bowel movements or reduced hard stools over 4 weeks in small trials, but a meta-analysis that pooled the Unique IS2 constipation trials found no significant increase in stool frequency. Persistent constipation, or any sudden change in bowel habits, is worth discussing with a doctor.

Is a heat-killed product such as StaiMune the same as live spores?

No. StaiMune contains inactivated cells of strain GBI-30, 6086, so it cannot germinate or make lactic acid in the gut. The trials on this page used live spores, and their results should not be assumed for an inactivated version, whose evidence is mainly laboratory work.
