Healthy blood sugar
A meta-analysis of eight trials in 423 people with type 2 diabetes found ordinary bitter melon preparations produced modest improvements in fasting glucose, post-meal glucose and HbA1c, with wide variation between trials. Everyone studied already had a diagnosis, insulin sensitivity was not measured, and none of the trials used Factor21.
Energy metabolism, animal data
The FGF21 and AMPK findings come from mice fed a high-fat diet for 12 weeks alongside a Verdure Sciences bitter melon extract. Liver AMPK and Sirt1 rose, while blood FGF21 fell and its receptors increased, which the authors read as better FGF21 sensitivity rather than more FGF21. No human study has measured FGF21 or AMPK with this extract.
Weight: not measured in the cited studies
Nothing cited here measured body weight, BMI, waist size or body fat. Pooled bitter melon trials that did measure them found no change, so there is no basis for a weight claim.
Mechanism of action
AMPK and FGF21 signaling, animal work only
In mice fed a high-fat diet, a bitter melon extract standardized to momordicosides raised liver AMPK and Sirt1 and altered FGF21 signaling. These are animal findings; the same pathways have not been measured in people.
Insulin-like glucose support
In laboratory studies, bitter melon compounds act on cellular glucose uptake. This is a proposed explanation from lab work, not something measured in the trials cited here.

423 adults with diagnosed type 2 diabetes, pooled from eight trials.
Fasting glucose fell 0.85 mmol/L, post-meal glucose 2.28 mmol/L, HbA1c 0.38 percentage points and total cholesterol 0.38 mmol/L. There was no effect on triglycerides, HDL or LDL. Results varied widely between trials and the authors call for further high-quality studies. Factor21 was not used in any of them; standardizing an extract to a marker compound does not make it the same as the mixed preparations pooled here.
